To begin therapy with Dr. Horton, you are required to complete intake paperwork which will also include several tests. These will help Dr. Horton understand your current situation.

Please DOWNLOAD the paperwork, complete it and bring it with you to the first session. If you choose not to download, please call the office to schedule a time to stop and pick them up prior to your first visit.
