Telephone Services

  • A five-minute telephone conversation with Dr. Horton. There is no charge for the first five minutes. There is a $1.70 charge per minute thereafter. This charge will be paid with Visa or Mastercard. *See Note*
  • A diagnostic, in office appointment, 80-minutes in length which includes several diagnostic tests to help pinpoint ways that Dr. Horton can be of help to you. The fee is $153.00
  • A 20-minute telephone consultation with Dr. Horton. The fee is $25.50, with the first five minutes being free of charge. *See Note*
  • A 35-minute telephone consultation with Dr. Horton. The fee is $51.00, with the first five minutes being free of charge. *See Note*
  • A 55-minute telephone consultation with Dr. Horton. The fee is $85.00, with the first five minutes being free of charge. *See Note*

*NOTE*

A telephone consultation for more than five minutes may constitute a therapeutic relationship. Therefore, for your protection, a signed agreement must be received prior to continuing beyond five minutes. You will be sent by mail or faxed a form outlining our mutual expectations and roles as well as confidentiality issues.
